DESCRIPTION
Desloratadine, the major active metabolite of loratadine, currently is approved by the FDA for use in patients with seasonal and perennial allergic rhinitis and chronic idiopathic urticaria. This drug is significantly more effective than placebo in reducing seasonal and perennial allergic rhinitis and chronic idiopathic urticaria symptoms, including nasal congestion. It appears to have a prompt onset of action in patients with seasonal allergic rhinitis and chronic urticaria. Clarinex also is safe and effective in patients with concomitant seasonal allergic rhinitis and mild to moderate asthma. How the efficacy of desloratadine compares with that of loratadine and other second-generation agents remains to be determined.

Desloratadine has a favorable safety profile. It is unlikely to cause somnolence and does not prolong the QTc interval or impair psychomotor performance or driving ability at recommended dosages. Clarinex appears to be less likely to interact with other drugs than fexofenadine.

POTENTIAL SIDE EFFECTS
Headache (14%), fatigue (5%), dizziness (4%), somnolence (2%), pharyngitis (4%); dry mouth (3%), nausea (5%); dyspepsia (3%), dysmenorrhea (2%).

CAUTIONS
  • Increased intraocular pressure, hepatic disease, renal insufficiency.
  • Not recommended for children under 12 years of age.
  • Desloratadine passes into breast milk. Therefore, a decision should be made whether to discontinue breast-feeding or it, taking into account the importance of the drug to the mother.
  • May be taken without regard to meals.

SPECIAL INFORMATION
Desloratadine is classified in FDA pregnancy category C. Clarinex was not teratogenic in animals exposed to the equivalent of more than 200 times the exposure in humans at the recommended daily oral dosage. In a separate study, an increase in preimplantation loss and a decreased number of implantations and fetuses were noted in female animals given the equivalent of approximately 120 times the exposure in humans at the recommended daily oral dosage. Reduced body weight and slowed righting reflex were reported in pups at doses equivalent to at least 50 times the exposure in humans at the recommended daily oral dosage. Desloratadine had no effect on pup development at oral doses equivalent to approximately seven times the exposure in humans at the recommended daily dosage. However, there are no adequate and well-controlled studies in pregnant women. Because animal reproduction studies are not always predictive of human response, desloratadine should be used during pregnancy only if clearly needed.

What is Desloratadine?
