DESCRIPTION
Premarin is an estrogen and is indicated for the treatment of moderate to severe vasomotor symptoms flushing, hot flashes) associated with menopause, vaginal or vulvar atrophy, and prevention of osteoporosis.

WHAT IS ESTROGENS?
A group of steroid hormones, including oestradiol (estradiol), oestriol (estriol), and oestrone (estrone), that control female sexual development, promoting growth and function of the female sex organs and female secondary sex characteristics (such as breast development and growth of pubic hair). Estrogens are synthesized mainly by the ovaries; small amounts are produced by the adrenal glands, testes, and placenta. Naturally occurring estrogens and conjugated estrogens (mixtures of natural estrogens obtained from the urine of pregnant mares) are used mainly to treat a deficiency of these hormones (resulting in lack of sexual development and absence of periods) and symptoms of the menopause. Estrogens are also used to treat some menstrual disorders and certain types of prostate and breast cancer.

Menopause, breast cancer (inoperable, in postmenopausal women and men), prostatic cancer, dysfunctional uterine bleeding, hypogonadism, castration, primary ovarian failure, atrophic vaginitis, prevention of osteoporosis.

POTENTIAL SIDE EFFECTS
Anxiety, blood clots in the blood vessels, breakthrough bleeding and spotting, breast pain, tenderness or enlargement, changes in sex drive, depression, disturbance in liver function, dizziness, fatigue, gall bladder disease, gut disturbances, such as nausea, abdominal pain, bloating, flatulence, indigestion, headache/migraine, irregular brown patches on the skin, usually of the face, leg cramps, premenstrual-like symptoms, rise in blood pressure, skin reactions such as rash and itch, steepening of corneal curvature which may make contact lenses uncomfortable, swelling of the ankles due to to fluid retention), vaginal thrush, weight changes.

CONTRAINDICATIONS
Breast cancer (except in selected patients being treated for metastatic disease), active thromboembolic disorders, known or suspected estrogen-dependent neoplasia, undiagnosed abnormal genital bleeding.

CAUTIONS
  • Hypertension, gallbladder disease, CHF, diabetes mellitus, depression, migraine headache, seizure disorders, hepatic disease, family history of breast or endometrial cancer, history of thromboembolic disorders, uterine fibroids, hypertriglyceridemia, hypercalcemia.
  • Hormone Replacement Therapy (HRT) should not be used by women who have cancer of the breast, uterus, or genital tract, thrombosis, or severe heart, liver, or kidney disease or by those who are pregnant or breastfeeding.
  • It should be used with caution by women who have had thrombosis or thromboembolism, migraine, diabetes, epilepsy, porphyria, fibroids, or tetanus. If pregnancy occurs HRT should be stopped immediately.
  • HRT may need to be discontinued before surgery.
DRUG INTERACTIONS
Tricyclic antidepressants, anticonvulsants, anticoagulants, oral hypoglycemics, tamoxifen, barbiturates, phenylbutazone, rifampin, corticosteroids, warfarin.

FOOD INTERACTIONS
Grapefruit juice.

HERBAL INTERACTIONS
St. John's wort, alfalfa, hops, saw palmetto, black cohosh, DHEA.

PREGNANCY AND BREAST-FEEDING CAUTIONS
Pregnancy category X; may decrease quantity and quality of breast milk.

SPECIAL INFORMATION
  • Use this drug cyclically or short term; prepare a calendar of drug days, rest days, and drug-free periods.
  • This drug cannot be given to pregnant women because of serious toxic effects to the baby.
  • Progestins recommended in non-hysterectomized women.
  • Premarin is derived from pregnant mare's urine; Cenestin from yams and soy. Although probably therapeutically equivalent, they are not substitutable by the pharmacist.
