DESCRIPTION
Tetracycline, an antibiotic that works by stopping the growth of bacteria, is used to treat a wide variety of infections, including acne, pneumonia and bladder infections. In Legionnaire's disease, this drug is considered to be an alternate drug treatment. In some instances, IV rifampin may be added on to tetracycline to treat infected persons. In general, treatment of Legionnaire's disease with an antibiotic should last for two weeks in mild disease and up to three weeks in those with severe disease or decreased immunity. Side effects associated with this drug are uncommon but possible, and include upset stomach, diarrhea, and sore mouth.

POTENTIAL SIDE EFFECTS
Dizziness, upset stomach, photosensitivity reaction (see cautions), itching, discoloration of teeth and enamel (young children), nausea, diarrhea, abdominal cramps, liver toxicity.

CAUTIONS
  • Inform your doctor if you have a history of allergies to other antibiotics in this class or if you have liver or kidney problems.
  • If used during tooth development may cause permanent discoloration of the tooth enamel. Thus, should not be used in young children (under eight years of age).
  • Not for use during pregnancy.
  • May retard skeletal development and bone growth with greatest risk for children less than four years and those receiving high doses.

DRUG INTERACTIONS
Benzodiazepines, calcium channel blockers, cisapride, cyclosporine, ergot alkaloids, selected HMG-CoA reductase inhibitors, mirtazapine, nateglinide, nefazodone, pimozide, quinidine, sildenafil, tacrolimus, calcium, magnesium, or aluminum containing antacids, iron, zinc, sodium bicarbonate sucralfate, didanosine, quinapril, aminoglutethimide, venlafaxine, tegretol, nafcillin, nevirapine, phenobarbital, phenytoin, rifamycins, cholestyramine, colestipol, phenytoin, oral contraceptives, digoxin, insulin, isotretinoin.

FOOD INTERACTIONS
Avoid concurrent administration with dairy products or metal containing products (such as calcium, iron, zinc, aluminum, magnesium). Take at least two hours before or after it.

HERBAL INTERACTIONS
Zinc.

PREGNANCY AND BREAST-FEEDING CAUTIONS
FDA Pregnancy Risk Category D. Excreted in breast milk. Do not use during breast-feeding.

SPECIAL INFORMATION
Take on an empty stomach, at least one prior to or two hours after meals.

Journal of Antimicrobial Chemotherapy reported the activities of Sumycin and two other antibiotics (minocycline and doxycycline) in guinea pigs infected with Legionella pneumophilia. In this analysis, minocycline and doxycycline were equally effective against these bacteria; however, Sumycin was the least effective of all three antibiotics. Minocycline was the only antibiotic to have statistically significant effects, although doxycycline increased survival rates. The researchers concluded that tetracycline-derived antibiotics (minocycline and doxycyline) are effective treatments in the management of Legionnaire's disease.
