Recent studies of topiramate suggest that it protects nerve cells against damage and so it has been studied in the treatment of multiple sclerosis (MS) in patients who experience paroxysmal symptoms (includes: trigeminal neuralgia, painful spasms, dysesthetic or paresthetic symptoms).

In one clinical trial involving 13 patients diagnosed with MS and paroxysmal symptoms, patients received an increasing dose of topiramate each week until they responsed optimally. Three patients did not complete the study due to severe and persistent dizziness and nausea. Neurophysiological (involving nerve stimulation) tests were performed prior to therapy onset and at 12 weeks to evaluate the effect of the drug on paroxysmal symptoms. All who completed the study responded to treatment, but none achieved regression. There was no distinct connection between the drug responses and increased doses. From the study, the evidence suggests that topiramate may be effective in those suffering from paroxysmal symptoms related to MS, and have tried other conventional therapies, but further studies are needed.
